Oral Rehydration Solution (ORS) is a life-saving therapywidely used for the management of dehydration, especially in diarrhoeal illnesses. However, in patientswith acute kidney injury (AKI) or chronic kidney disease(CKD), including those with hypertension, oliguria, or fluidoverload, standard ORS formulations may cause harm. These patients often present with impaired sodium andpotassium handling, low urine output, and increased riskof volume overload or hyperkalaemia. This reviewsummarises the limitations of conventional ORS in thispopulation and provides evidence-based, stage-wiserecommendations for renal-safe ORS formulations thatconsider fluid status, serum potassium levels, GFR, andcomorbid conditions like hypertension.
